Attention: All Service Providers interested in working with Coordinated Family Care
In order to establish a partnership within the local Children's System of Care and Coordinated Family Care forms are required to be filled out and returned to Salma Harris at providers@coordinatedfamilycare.org. You are also required to attend a New Prospective Service Provider's Meeting. These meetings are held on the last Wednesday of the month at the end of each quarter in 2022. Here are the meeting dates & times 10am-12noon:

December 14,2022 from 12noon-1pm: Resource Fair held for newly signed on Providers (last 1-2 years) more information to follow.
*For the month of December 2022 there will not be a New Prospective Service Provider Meeting. The next meeting will be posted in the New Year. A Virtual Resource Fair will be held on 12/14/22 from 12noon-1pm. Special invite with a focus on recently signed on providers. Direct communiation will be made with those providers. Thank you for your copperation and understanding.
The Middlesex County “Local System of Care” Prospective Service Provider Meeting:
The purpose of the New Provider Meeting is to:
Learn about the various requirements and expectations to partner with Coordinated Family Care (Middlesex County CMO), Mobile Response Stabilization Services (MRSS), and the Family Support Organization (FSO of Middlesex County).
All interested parties must register via the Zoom Link once provided prior to the date (please don’t wait until the last minute to complete this as an approval process takes place).
