Job Description: Supports and implements the day to day operations of the organization through the supervision of Care Management Supervisor staff. Ensures that Care Management teams are effective in the provision of services and adhering to all relevant regulatory standards. Job Description: Provides coordination and/or the direct provision of assessment, service planning and care management services to Middlesex County families that care for youth who present with moderate to very high levels of behavioral and/or emotional needs, and/or developmental disabilities. Primarily responsible for assisting families in overcoming obstacles by utilizing and building upon their strengths and vision.
