Operations Manager Assistant

Education/Skills: BA in Human Services– no prior experience. Computer skills or previous knowledge of CYBER. Interpersonal skills. Persistence, organizational and time management skills. Must be able to drive agency vehicles. Opportunity to be eligible for CM position after 6 months working in the role.

Job Description: Provide support to Care Manager teams under the supervision of the Operations Manager. Job duties to include, but not limited to: Finding the providers and the subsequent documentation that goes with that (referral form, uploading it into youth files etc.) File reviews. Transfer forms for Care Managers. Uploading and Updating SharePoint and CYBER as directed. Scheduling specialty evaluations. Work in office In-person as needed/requested. Office minimum 1 day per week (3.5 hours) or more as needed/requested. Field visits, transports as needed/requested.

Care Manager (Bilingual, Spanish)

Education / Skills
Bachelors Degree in a Human Service field. At least one year post undergraduate/graduate experience providing family and youth services within a non-profit social services setting. Excellent communication, facilitation, assessment and system negotiation skills, creativity and enthusiasm are essential. Ability to work as a team. Ability to demonstrate professionalism at all times. Bilingual (Spanish) required.

To provide coordination and/or the direct provision of assessment, service planning and care management services to youth and families in a manner that is respectful, dignified and that enhances family self-sufficiency. A primary responsibility is to assist families in overcoming obstacles by utilizing and building upon their strengths and vision. The Care Manager is responsible for ensuring that the mission of the organization is carried out through the work that it is charged with.
